NOTICE OF FORMATION OF A DOCTORAL DISSERTATION COMMITTEE

To create an HTML form, please fill out the fields below. You will need need to use a recent version of Internet Explorer to print this form correctly, since older versions and Netscape are too buggy. The page setup should have no headers or footers, and 0.75 inch margins.

Now go into the Page Setup under Internet Explorer's File menu and blank out the headers and footers. (You may want to write down the old settings first.) Set the top and bottom margins for 0.25 inch and the side margins for 0.75 inch. Go in the View menu/ Text Size and select medium text size (for a 800 by 600 screen).

Next press the "Format" button below and then use your browser's Print button (or the corresponding item in the File menu) to print out the formatted form.
